My doctor said because the pits and fissures of my back teeth are too deep (i have 4 medium back caries), i must fill in the teeth first and then apply sealants on the surface to prevent further decay. can i do sealants when having caries already?

If the caries are too deep for sealants, then you need fillings, no sealants required. Sealants are used on teeth with little if any caries.

Thats a little silly - If they need fillings a proper one will include post sealing of the margins. Definintely would not pay extra for them
