What does this mean: the right adnexa shows a smooth well defined upper margin with no obvious opacification of adnexa (abrupt cuff-off) the left adnexa shows contrast opacified tubular structure but no free peritoneal spillage seen?

With the description provided, it is consistent with normal spill of contrast ("no abrupt cut-off") from the right Fallopian tube, but "no free spill" from the left adnexa makes the left tube essentially non-functional for achieving pregnancy. With the closure of the left tube, it may be appropriate for a diagnostic laparoscopy to be done to evaluate chances for pregnancy.
