Bleeding with clots can obstruct the cervical canal, causing significant pain. Thickened endometrial lining from twin ovulation may cause heavier bleeding with clots. If pregnancy is possible, obtain a serum HCG test to be certain about pregnancy, which can occur outside the uterus (ectopic). If positive, see your gyn provider as soon as possible.

What you described suggests loss of pregnancy. If the bleeding has not subsided, you should to the emergency department of a hospital for urgent care. Wish you good health!
