Rather infrequent but can occur when vascular risk factors are present. Women with diabetes, hypertension, hyperlipemias, and congenital heart disease might be at risk. As noted, toxemia of pregnancy would be a cause, albeit difficult to anticipate in many unless blood pressure is monitored. But in most pregnancies, we neurologists are hardly ever called.
