I would first make an appointment for the dentist to see him. Very often unusual mouth odors are dental in origin. Dentist can also see if there looks to be erosions on teeth from gastroesophageal reflux that could also be the origin of the odor. Has he been on any new medications lately? These could also be cause of unusual mouth odors. Start with dentist and good luck...

Monitor his temperature. A sinus infection, GERD or mouth infection may be possible causes. Discuss nasal saline and a probiotic with your PCP. Keep him well hydrated.
