Second opinion: Not clearly feasible to pin down a precise diagnosis based upon your description, but it appears that interventions have been fruitless so far. Find a new neurologist, hopefully a headache specialist, consider a completely new approach. (Incidently, biofeedback and acupuncture have provided help for some.)

Description: Your litany of symptoms, while daunting, is only part of the information needed. As treatments have been ineffective, time for comprehensive reevaluation. Suggest you invest in an evaluation at the Oro-facial Pain Management Departments at U of Kentucky or U of Pennsylvania. These are two of the leading programs in the US.
