Pus in sperm: The treatment is to find out the causes of both issues. At your age I would suspect you have a sexually transmitted disease. Most likely GC but can be many things. And in your part of the world theses may be resistant to usual antibiotics. Discuss with your team. Today would be good!

Recommend urology ev: If you have low sperm count which you do and pyospermia(pus in semen ) I would recommend a semen culture first to determine if bacteria is growing in semen to get appropriate antibiotics as this can drive down sperm numbers. I would recommend 4-6 weeks of antibiotic and then repeat semen analysis in 3-4 months. But see a urologist for definitive evaluation .
