i am 21 years old, average built. when i sleep, my heart rate goes below 40 bpm. do i need to be worried? and get checked out?

Low pulse: In someone your age this can sometimes occur, especially when sleeping. If you don't have any cardiac type symptoms, I wouldn't worry about it. You might consider getting a TSH test to rule out hypothyroidism.

I am concerned by the phrase”below 40 bpm. I would strongly advise that this be confirmed by monitoring of the rhythm during sleep. Hypothyroidism usually slows the heart rhythm while asleep or awake.

Sphincter tone: We have 2 sphincters that prevent stool from coming out. One is internal, the other is external. As long as one is working, stool should not come out. If you have had children, you could have damaged these sphincters. Diabetes, ms, infectious diarrhea can cause incontinence.

Cardio conditioning: There are many causes of cardiac inflammation--infections, drug/chemo, radiation effects, excess alcohol, rheumatologic diseases, uncontrolled diabetes etc..While exercise itself does not get rid of inflammation in the heart, it may improve heart function and general health. It can help lower cholesterol/sugar, blood pressure and thus better heart-health. Other diseases must be treated. Good luck.
