Possible emergency: Torsion or twisted testis must be excluded if pain developed suddenly. This is an emergency as blood supply to testis is cut off. Epididymitis &/or orchitis are painful inflammatory conditions with swelling of testis. Onset of pain is slower than with torsion. All cases of acute onset of testicular pain must be considered ae torsion until proven otherwise in Emergency Room. Good Luck.

Painful testicle: In a 25 year old, my first thought is that there is an infectious or inflammatory process causing the pain. Typically this is slow in onset compared to testicular torsion, which is usually of sudden onset. Your best option is to see a urologist, who can determine the optimal treatment for you.
