Could severe stress and anxiety mimic heart attack symptoms on and off for months? have already had heart tests and cardiologist said all good?

Yes: Panic attacks can result in hyperventilation with chest pain and tightness, and "air hunger". Glad your cardiologist has cleared you. Now, see a cognitive-behavioral therapist. Also, might consider biofeedback therapy as an additional intervention.
