Subclinical hypo : The decision to initiate treatment in subclinical hypothyrodism depends on the symptoms as well as findings on laboratory testing. Assuming the free T4 is normal and TSH is 9.5 and the fasting sugar is 155 (diabetic range), most experts would agree that treatment with thryroid replacement therapy be initiated. Autoimmune thyroiditis may lead to permanent under production of thyroid hormone.
