Doctor can check: If there appears to be 3 testicles, a primary care doctor can examine to see what the third one is (because it is not likely to be a true testicle). The doctor might order an ultrasound scan to see what is in the third one.

Triorchidism: Polyorchidism is rare congenital anomaly consisting of more than one testis. Triorchidism is three testes. The majority of extra testes are located in the scrotum (66%) followed by groin (23%) and abdominal (9%) position. Most cases of polyorchidism are asymptomatic, and are discovered incidentally usually by ultrasound. The most common form shares vas deferens and epididimis with normal testes.

Testes: Although it is possible that a man could have three testes, it is far more likely that he has two normal testes and a large spermatocele (approaching the size of a tetis). Have an ultrasound and physical exam by a urologist to confirm.
