Severe back pain: Excruciating low back pain in the absence of an injury or acute strain is unusual. A kidney stone attack comes to mind. It often starts in the flank,radiates downward and also may radiate toward the groin. If you are in extreme distress, I recommend proceeding to the emergency room where prompt evaluation, appropriate testing and treatment are available
