Evaluation needed: Many times with crohns this represents a high grade partial bowel obstruction which needs evaluation. Initial work up would include a ct scan of the abdomen and pelvis with PO and IV contrasts and a colonoscopy. Hope this helps.

Many reasons: It sounds like you have gas in the L side of the colon. The gas could be the result of the Crohn's disease causing food not to be absorbed. So the bacterial in your intestine digests the food producing gas. Flagyl may kill some of the bacteria, but they quickly return. Yes, it could also be from a partial bowel obstruction. If it was a complete bowel obstruction you'd have severe pain
