Under tongue there is what looks like flaps and one of them is red and has a blister what could this be?

Depends: If it is an ulceration it is probably a canker sore. If it is a a bubble filled with with a clear looking fluid is probably a plugged up minor salivary gland. If it looks like a blood blister it is probably due to some minor trauma. If it lasts more than 5-7 days you should get it checked.

Herpes simplex: The flaps are okay anatomic structures, . The blister is probably a herpes infection. Most people have the herpes virus and outbreaks occur. If it hangs around for more than ten or twelve days, see a dentist.
