Bacteria/yeast: Skin can be malodorous for a variety of reasons. In a 5 yr old, there may be an overgrowth of yeast or bacteria on the skin. Try washing daily and using an antibacterial soap or cream/ointment for fungus containing miconazole. You can also apply hand sanitizer to reduce skin bacteria and reduce the odor. Make sure your 5 yr old is examined yearly and has all their vaccines.

Endocrinologist: The symptom you mention is not common for a 5 years old. One of the reasons is a endrocrinologic condition. In other words, your baby may be producing sexual hormones that shouldn't be produced untill puberty. I recommend you to visit a endocrinologist
