Fluid in space: The knee joint has a cushion like a poorly inflated balloon filled with fluid that sits between the big bones. If that balloon reacts to a triggering event by filling with more than normal fluid it is called an effusion. Unlike many areas of the body with swelling an effusion is within this special space that prevents it from loosing excess fluid quickly.Once there it usually stays for a while.

Swollen knee: A layman's term for this is "water on the knee". This means that fluid has collected around or internal to the knee.
