Sometimes when your intestines are full the u leak some poop if u try to hold it.is it bad to stay with an underwear which contains poop for 21 hours and also does it cause cancer if the poop spread all over the butt hole?

Unsanitary: What you described is unsanitary and you would be smelling of poop all the time. There is no risk of cancer but you could develop a skin rash from long exposure to bacterial laden fecal matter. If it is a frequent occurrence, carry spare underpants with you and clean yourself up as soon as possible. Infection could travel up your vagina and may be urinary bladder. Get HPV vaccine.
