Together: The reason for a physical together with routine labwork is that both are needed to determine if there are any underlying problems. Taken at different times, there is less correlation and therefore it is not as useful for diagnostics or to follow chronic conditions. You could do the labs now, but the Dr would probably want to repeat them when you go in for a physical. Better to reschedule both.

Lab request: Usually the doctor determines if lab work is needed. It sounds like you want someone to order your lab work and then you would decide if you needed to see the doctor based on the lab results. As soon as another doctor orders your labs, that provider has just taken on legal liability. The situation is not as simple as it may sound. Your doctor may perceive it as working around him or her.
