Skin care: Keep the diaper area clean and dry, use frequent diaper changes and a standard gentle diaper cream. If the rash spreads or is associated with increased redness, pus or fever, please have an evaluation soon. Continue a topical antibiotic such as Bacitracin or the equivalent three times daily. Discontinue the anti fungal cream, and please follow up. Other possible causes include an insect bite.

See her doctor: It's best to have the rash/spot checked by her doctor, either in person or via telemedicine so that proper treatment can be recommended. The treatments are different depending on whether this is an irritation, eczema, bacterial infection, fungal infection, injury, or insect bite.
