Anal fissure?: The most common cause of painful stools is an anal fissure that then causes spasm of the anal sphincter. This can be easily diagnosed by an experienced physician examining the anal opening. Special ointments with diltiazem, Nifedipine or Nitroglycerin can often get the sphincter to relax and relieve the pain. Occasionally surgery is necessary.
