In order to find a diagnosis is necessary much more information about your symptom or other associated symptoms, as well as a physical examination. You can try avoiding soaps.

Depends how it looks: If your hands get wet often (frequent dishwashing for example) this could be a fungal infection (usually redness, bumps)a clotrimazole cream can help. if you sweat easily it could be a condition called dyshidrosis (usually blisters)- hydrocortisone cream can help. Video visit would be helpful, plus more information about when it occurs
