Multiple reasons : Italy may have been hit hard with COVID 19 partly because of a lot of travel between China & Italy (that is known to be true) & because Italians love large family gatherings & church worship services where there are many people in close proximity, & the government did not begin social distancing right away. There may be other factors with coexisting health issues (cardiovascular, diabetes)
