My son is three years old,non verbal recently he has bladder problems,no fever,not sick at all...he struggle to urinate,he keep his pee in for way to long (more then six hours) after a bottle...when its time to urinate he screams like he is in pain?

Assume there is pain: Though it may be a difficult evaluation my first thought would be the kid has an outlet obstruction of his penis or a urinary tract infection. Putting him in a warm tub bath to pee can relax his belly & may make it easier, but he should be seen to sort this out.
