Decisions are hard: There tends to be much psychological pressure among close relatives when it comes to organ donations among them. Also, a 75 year-old kidney has been in use for 75 years, which feels like a long time. Would somebody want to receive a 75 year-old kidney? Taking one from Grandma would leave an elderly woman with only one remaining old kidney. Decisions are hard.
