It depends...: ...on initial infection or recurrent outbreaks, treatment goals, and other factors. For longstanding infection, valacyclovir or acyclovir by mouth are effective in reducing frequency and severity of outbreaks. Valacyclovir reduces risk of sexual transmission to partners; acyclovir hasn't been studied. Topic treatment (creams, ointments) don't work well. Nothing else works (see comment to follow).
