Depends on details: Usually means the foreskin is a bit long or tight and causes itself to balloon during peeing because urine coming out of the urethral meatus is faster than the urine leaving the foreskin end of the penis. His pediatrician can examine and decide what to do. Parents can make a video of peeing and ballooning to show doctor, but delete the video afterward, don't share it, don't upload to the cloud.
