Not necessarily: If there is associated proteinuria, the microscopic hematuria is most likely from progressive glomerular damage rather than infection. I'm not sure that you can be sure that it's your kidneys that are hurting. Your nephrologist, who knows all your findings, can advise you best and can investigate your back or flank discomfort.
