Dr. can check, but..: Until one's doctor has evaluated, the preliminary theory on what causes this is usually: the guy is pushing his penis too far into the woman even though the woman says it is hurting her when his penis goes in that far. It could just be simple physics, or not. The couple can go together to the doctor so that the guy can explain what he is doing, since that helps the doctor's assessment.
