Rarely: I have seen patients in very impoverished situations that suffered from a rare disease called noma (cancrum oris). It affects mostly malnourished children who are immunodeficient, and they develop gangrene. In the US it is possible to get osteonecrosis of the jaw, but it is usually not fatal.
