Notify MD: You should not wait for your next appointment but should notify your physician or nephrologist that your bp is uncontrolled as this will do more damage to your kidney status. Don't increase your doses of bp medications on your own as some(like ACE inhibitors ,ARB's or diuretics) may do harm to kidney function. Get help from your physicians.

Best : Would be to let your nephrologist or PCP know. For as important as blood pressure readings are more important are your symptoms. Your blood pressure is high, but is rather predictably high.

Best advise: Will come from your nephrologist, who likely has specific parameters for your blood pressure. In general, we do not recommend going directly to Er for abnormal BP readings, but rather symptoms corroborated with BP readings.
