I need a fit for duty form filled out asap for my employer for asthma ive had my entire life, stating i cannot work in the smoking bar we just added. can i go to urgent care for that? i do not have a primary care physician in nevada yet.

Wrong place to ask: This is the public HT site where volunteer docs answer questions about health problems. We do not offer treatments or notes. Such notes are legal documents which must stand up to court review on occasion. Any doc who furnishes one without a direct patient relationship, notes & exam can lose or have their license revoked. Urgent care may offer this service but I would call first to see.
