Two Doses: The vaccine comes as combined MMR now w/ the rubella portion (german measles mixed w/ mumps & measles) in 1989, young people in the usa, began getting 2 doses of MMR after increased outbreaks erupted on single dose regimen. Single dosing not protective enough. Those born before 1956 may have natural immunity. If born between 1957-1970's, you may have had only the single series & be at risk.

Depends: For most it will be 2 doses no less than a month apart given after the first birthday. For women in childbearing years, a screening test may suggest the rubella antibodies are low and a dose between pregnancies is often given, regardless of prior doses.
