Hard to know: Infants & toddlers have a great capacity to shunt surface blood flow deeper to avoid heat loss. This may be seen as feet cooler than their trunk. It would be more likely in a cold environment. Whether or not this explains this or not would require a direct exam & review of the child's other vital signs/findings.
