See a doctor: Vartious infections can do this, including STDs if at risk (e.g. herpes, syphilis), Bartholin gland abscess (sometimes caused by gonorrhea or chlamydia). If the "white spots" are open sores, herpes is likely. A severe yeast infection (thrush) also is possible. This really should not be treated without knowing the cause.
