37 day cycles: imply ovulation around day 23 of cycle rather than day 14 as seen in 28 day cycles. This reduces the number of ovulation per year from 13 down to 9 or 10. This lowers the chances of pregnancy just based on this alone. Also likely other hormone/insulin resistance issues that further reduce chances of achieving and continuation of pregnancy. Testing can be done to improve chances.
