Can an ear infection cause a rash all over face? doctor said hes not contagious but it looks awfully like impetigo and not just an ear infection.

No...but: The germs involved in childhood ear infections are local. The germ cannot escape that space & move to the face. If he truly has a blister like rash on his face there can be germs on the skin surface that trigger such an infection separate from the ear infection. If the rash is not blister like it is not impetigo.
