Both check stability: For varus test, the thigh is stabilized by a hand just above the knee, and pressure is exerted laterally just above the ankle. Pain or laxity at the knee assesses medial collateral ligament injury, and to some extent, the ACL, PCL, and minor ligaments. The varus test is just the opposite and is looking for laxity or pain on the lateral side of the knee. Other maneuvers assess ACL and PCL injury.
