Goals and context: Are missing. Not commonly used in US, I only can think of few circumstances that will make sense. At this doses usually immediately release opiates are backed up with sustained release opiates. Other than for palliative reasons, not a long term solution regardless of medical condition. Talk to prescriber in order to understand goals of therapy.
