Have question for my mom. 54 yo, entire colon removed a few years ago. severe pain where pouch is and only water coming out. what could be the problem?

Multiple things: Sorry your mom is going theough this. If by pouch you mean ostomy bag then, with pain, this might indicate (but not limited to) a narrowing of gastrointestinal tract around the pouch, a fistual (abnormal connection between body surfaces), or an infection. It is necessary to see her doctor without a delay for further evaluation.
