18mo son has diarrhea today, he vomited about 1 hour ago, and now a diarrhea diaper with a (single) long black fibrous bit in it. ?

Diarrhea: There is a possibility of a viral infection - stomach flu. This is also know as gastroenteritis. This is usually self limited and will resolve in 2-3 days with fluids such as pedialyte. The dark fibrous bit could be something that he ate. I would monitor his bowel movements and see if the dark fibrous bit recurs. If so, I would consider evaluation by the pediatrician.

No: ...Although it is normal for breastfed baby's stool to be quite loose. Rule of thumb: if the weight gain is appropriate, it isn't diarrhea no matter what you find in the diaper, or how often.

Yes: All foods, including formulas, are digested differently. Making a change between formulas will cause the intestines to digest different nutrients. This may cause temporary constipation, diarrhea, or both (or neither) until your digestive system adapts. Many people find that with changing formula their child gets looser stool and a diaper rash in the short term; should resolve in a week or two.

More info: "chicken bumps" are more commonly seen on the arms and legs and do well with cod liver oil (orally). On the face...Depends on the age. If a newborn is one thing. Older children i would want to see the rash before offering suggestions. In general there is a great cream..Called schmoove. Can be ordered from maker. See a doctor: This is not normal and needs a rapid diagnosis, and treatment based on the diagnosis. You have an inflammatory condition of the colon and this could be infectious or inflammatory bowel disease or other. Do not wait. See somebody asap, and hope you are better soon.
