Testicular lump: No, the lump on the testicle does not mean gonorrhea. But he definitely needs to have further evaluation to establish the correct diagnosis. He probably requires an ultrasound of his scrotum.

Not particularly: The lump is likely an incidental finding unrelated to the GC. It may not have any significance other than being a benign finding (appendix testis) or could be something wrong with the testis.
