How old is your son?: It looks like urethritis, inflammation of the urethra, the tube that conveys the urine /semen out, he will need assessment, urinalysis, and may need antibiotics, meanwhile ask him to drink more fluids, that will help flush the urinary tract, hopefully the soreness will abate momentarily, wish you both wellness
