Yes: This is critical. Even one blast cell on peripheral smear is extremely serious. Some automated cell-counters using image analysis may mistake them for normal lymphocytes or monocytes. There's still no artificial-intelligence substitute for the human eye. If a hematologist suspects myelodysplastic syndrome, leukemia or some other myeloproliferative disease, they will look at the smear themselves.
