Rotator cuff tear ?: Rotator cuff tear is a possibility. An MRI of the affected shoulder would show. Consult to your docor.

Weakness of muscles: You need to be examined by orthopedic doctor with tests like MRI of shoulder- You have not mentioned which shoulder? Most probably rotato cuff problem- many muscles may be involved.
