Nerve or muscle: Depsending on the tooth extracted,. Some teeth have nerves running very close to their roots and can be traumatized by pressure or inflammation from the extraction can irritate the nerve that controls sensation to the lip. The twitching is the nerve irritation.

Neurologic: Probably alteration to the nerve that supplies motor function to your lip muscles. See the General Dentist or specialist Oral Surgeon the treated you fro a post-surgical reevaluation. Referral to a Neurologist may be in order.
