There are many: causes that would explain your symptoms. GI causes such as gallstones come to mind. But there might be non GI causes such as hormone imbalances, nerve problems, or even occult infection or malignancy. You need to see a doctor for full evaluation, correct diagnosis, and proper treatment.

Gastritis : You may have inflammation of the stomach caused by the nicotine and caffeine. If you stop smoking and decrease your caffeine intake I think everything will resolve. However if this is not possible try an H2 antagonist such as Zantac,that my help. Good Luck
