Worry about infectio: You may have cord compression, epidural abscess, discitis or osteomyelitis of your spine. Would recommend going to ER for blood cultures x 3 sets (separate times and separate sites), CBC, cmp, esr, crp and spine imaging and a thorough physical exam by a physician (not midlevel). Make sure they do not give you antibiotics before doing the blood cultures.

Go to the ED: You should go to the ED. The longer you have neurological symptoms (such as erectile dysfunction) the less likely they will return to normal if you have spinal compression and not just a bad episode of arthritis pain causing your symptoms. The night sweats are concerning for an infection causing compression as compared to a herniated disc or other degenerative changes.
