Always be concerned: Watch for any lump which is enlarging over time especially in a post menopausal woman. I would be concerned if the mass is fixed, not mobile, firm, rather than soft. See if skin is retracted or red around the lump. Finally, if there is a bloody discharge on one breast i'd be concerned.
