Recommend evaluation: Anytime there is severe pain like you describe that persists for more than a few hours deserves an evaluation with a more comprehensive history and exam. With both lower abdomen and back involvement, one thinks of a kidney stone or kidney infection, but there are many possibilities. I hope you feel better soon.
